  2. Complete a one page Agri Fact Find - Download Agri Lending Fact Find
  3. Completed loan application form
  4. Statement of direct payments
  5. Notice of assessment from revenue (acknowledgement of form 11 or Chapter 4 self- assessment statement)
  6. Bank statements for the last 3 months
  7. Evidence of non-farm income
  8. The Last milk statement for the previous year
